Project Embark: supporting people experiencing homelessness and mental illness to access the NDIS .
Embark is a project funded by the NDIA through the NSW Ministry of Health to support people with a mental illness, experiencing or at risk of homelessness, to access the NDIS.
Embark will work with Local Health Districts, Specialty Health Networks, mental health services, homelessness services and community services to undertake outreach activities to engage people with a mental illness who are experiencing or are at risk of homelessness.
Embark can help anyone who:
- is homeless or at risk of homelessness;
- has a mental illness; and
- is in the Sydney area
Embark will take referrals from any source, including the person themselves, carers, families, treating clinicians, community members and support services.
If you would like Embark to assist someone experiencing or at risk of homelessness who has a mental illness, please Make a Referral Here or call Nick Baistow on 0481 748 101.
